The Welfare area in Qeqqata Municipality is seeking a caseworker with a referral function for the Authority area for the Elderly and Disabilities, to start as soon as possible or by agreement.
The position is located in the authority area, where you will be part of a professional team that works with assessment, case processing, and follow-up on the municipality's services for elderly citizens and citizens with disabilities.
About the position
As a caseworker with a referral function, you work with referrals and case processing in accordance with applicable legislation and the municipality's quality standards.
You will help ensure that citizens receive the right assistance and support based on an individual and professional assessment.
The work involves collaboration with citizens, relatives, service areas, and interdisciplinary partners, including home care and home nursing, nursing homes, therapists, and the healthcare system.
The position is under development, and the tasks can therefore be adjusted according to needs and the scope of the assignments. The position is expected to be involved in servicing the entire municipality within the elderly and disability sector and is located in Sisimiut.
The position reports to the department head for the Authority Area for the Elderly and Disabilities.
Work tasks
Your tasks will include:

Case processing and assessment according to applicable legislation and quality standards

Preparation of health and social professional assessments and decisions

Processing of applications for support and municipal offers

Coordination and follow-up in citizen cases

Gathering relevant health and medical information

Conversations with citizens and relatives

Interdisciplinary collaboration with internal and external partners
The visitation may include, among other things:

Home care and round-the-clock home care

Activity offers
-
Meal arrangement
-
Transport to activity offers and disability transport

Training and respite stay
-
Elderly and disability-friendly housing
-
Companion scheme and support person scheme

Housing units and other social support measures
We expect that you

Has knowledge of health and social legislation in the area of elderly and disability services.

Has experience with case processing or authority work

Can work in a structured manner and maintain an overview in a busy everyday life.

Has good collaboration skills and can work interdisciplinary.

Can communicate clearly both in writing and orally.

Works independently and takes responsibility for own tasks.
Professional qualifications
We expect that you:

Is a social worker or has another relevant health or social education; further training as an assessor would be an advantage.

Has experience in the social or health sector

Has good administrative and organizational skills

Speaks and writes Greenlandic and Danish (an advantage)

After 1 year of employment, training as a visitor or training as a dementia coordinator can be offered if the employment is covered by the PPK agreement.

Salary and employment

Salary and employment conditions according to the applicable agreement for AK caseworkers or according to the relevant agreement based on educational background, e.g., the PPK area. Opportunity for a holiday travel allowance every other year for you and your family.

The position is designated as full-time, but there may be an opportunity for part-time work upon application.

The employment is subject to a 3-month probation period.
Housing
Housing for staff can be allocated according to applicable regulations.
Moving assistance and relocation travel is provided for you and your family if you are not already residing in Sisimiut.
